Client-Practitioner Agreement

The Empowered Health Signature Coaching Plans Include:

  • 2 to 2.5-hour Initial Strategy Session with session notes and a detailed preliminary, self-healing action plan -- this is where all the foundational work is set in motion for the journey ahead

  • Review of recent, relevant labs (no older than 6 months)

  • Initial HTMA (Hair Tissue Mineral Analysis) from Trace Elements Labs (to be reviewed when results are received)

  • Follow-up Coaching Sessions @ up to 90 minutes each max over a maximum period of 18 months. Additional fees will be incurred for sessions that exceed this time limit. Clients are always given the choice to allow sessions to go over in time or not.

  • STANDARD PLAN -- 14 follow-up sessions spread out over a max time period of 18 months

  • ULTRA PLAN -- ULTRA PLAN -- 29 follow-up sessions spread out over a max time period of 18 months, typically scheduled every 2 weeks (or more frequent if strategic)

    NOTE: One can choose to upgrade from the STANDARD to the ULTRA PLAN at any time during the course of the year for an additional investment of $5500.00. Additionally, the number of coaching sessions already used in the STANDARD PLAN will be subtracted from the maximum of the 29 additional coaching sessions provided in the ULTRA PLAN.

  • All the time spent on your case outside of sessions for planning and post-session write-ups 

  • Ongoing messaging support per the messaging guidelines, which are fully explained below

  • Ancillary supports (video, handouts, checklists, etc.)

  • Access to professional-grade supplements

NOTE: The investment in this coaching plan does not include the cost of supplements, lab fees, and other health supports that may be presented as interventional healing options.

In our work together, my role is to help you become your own best SELF-HEALER. To do this, we'll take a deep dive into helping you to optimize seven pillars of wellness, which include:

  • Clarity of wellness vision

  • Identifying and addressing root causes and barriers to healing and life success.

  • Self-care strategies

  • The power of daily habit

  • Environments (internal and external)

  • Knowledge and skill

  • Mindset for healing and success

AND...in doing this, some of the more specific methods and focus of our work will include:

  • Tapping into the power of the subconscious mind:

  • PLEASE NOTE: A large part of our work will entail applying the Wellness Code Energy Testing & Transformational Coaching System. This modality will be a significant component of most if not all, coaching sessions to varying degrees. It’s important that all clients understand this. 

  • Lab testing (when appropriate and used conservatively)

  • Targeted supplementation (when appropriate and used conservatively)

  • Gut health

  • Mitochondrial health

  • Daily tools for nervous system regulation (this will be a BIG focus with emphasis on functional and therapeutic breathwork)

  • Harnessing the power of neuroplasticity and neural rewiring

  • Exploring passions and purpose

  • And more!

Payment Options:

There are three payment options for The STANDARD or ULTRA version of the Empowered Health Signature Coaching Plan:

Option #1: Pay in full (BEST value)

Option #2: Initial down payment followed by monthly installment payments. This option adds a 10% financing fee to the total price. If a client opts for the payment plan option but payments are not honored/paid per the stipulations set, all services will be stopped, including messaging support, until the situation is fully rectified.

Option #3: Pay Pal invoice for the full amount with financing option to those that qualify (which is determined by Pay Pal)

Due to too many default payments in the past, I no longer offer extended payment plans. I can, however, send a PayPal invoice to those who request it. Pay Pal offers six months of free financing to those who qualify. You can also consider using a credit card or exploring loan companies that offer personal loans.

NOTE: The signature plan pricing does NOT include the price of supplements, lab fees, and other health supports that may be presented as interventional healing options.

ALSO NOTE: One can choose to upgrade from the STANDARD to the ULTRA PLAN at any time during the course of the year for an additional investment of $5500.00. Additionally, the number of coaching sessions already used in the STANDARD PLAN will be subtracted from the maximum of the 29 additional coaching sessions provided in the ULTRA PLAN.

Refund Policy (after enrollment in a coaching package/program)

Due to the extensive screening process for qualified clients combined with the amount of time, energy, and commitment to the healing process dedicated to each client within the scope of the services I provide, I limit the number of clients I take on at a time.

Additionally, there is a strong correlation between a client’s level of commitment to a health and wellness coaching program and their success in the program. Considering these factors, there is only a small window of time in which refunds will be granted for a coaching package/program.

Each client is given three days from the date the invoice is paid to withdraw from the agreement and request a full refund for any fees paid. No refunds will be granted after this 3-day grace period.

Under no circumstances will Jennifer or Empowered Health LLC refund any payment for the coaching program and/or time spent on the client's case after the 3-day grace period.

Putting a Plan on Hold:

I fully understand that life can sometimes become complicated and prevent us from keeping commitments that we originally agreed upon. Considering this, clients are allowed one opportunity to put a coaching program/plan on hold for up to 6 months with no penalty. When the program/plan is on hold, no time will be spent on the client’s case, responding to messages, or scheduling coaching sessions.

To place a coaching program/plan on hold, a client needs to submit a message via the portal requesting the hold and briefly explaining the reason.

When a plan is placed on hold, the expiration date is extended for the duration of the hold.

My Right to Terminate a Coaching Plan / Refund Policy:

I reserve the right to terminate the working relationship in the following scenarios:

  • A client does not adhere to all rules of engagement that have been explained within all waivers sent

  • I deem the dynamic of the client-practitioner relationship to not be conducive or healthy for either party

If I (Jennifer) need to terminate the client agreement and/or cease providing services, the client will be reimbursed for any services and/or time that have been paid for but not yet rendered. 

No refunds will be issued for services rendered and/or time spent on the client's case. Additionally, time spent on a case (which will be subtracted from the refund amount) will be calculated on the current hourly rate for stand-alone coaching sessions. 

In this scenario, the refund is calculated by taking the total amount of time spent on your case both inside and outside of coaching sessions (not including session overages already paid), multiplying it by the current hourly rate, and then subtracting that amount from the cost of the coaching program (not including financing fees for a payment plan).

Expiration Guidelines For The Signature Coaching Plan:

Both the Standard and Ultra Signature Coaching plans expire 18 months from the date of purchase unless an extension is requested. See the section marked "putting a coaching plan on hold."

Payment for Pre-Paid Supplements, Labs, Shipping, and Handling Fees:

A client case often requires that I order and drop-ship labs and/or specialty products/supplements on their behalf through our practitioner accounts. Because of this, you will be asked to provide a valid credit card to which these fees will be charged. The exact amount of the charges will be communicated to you before charging your card, and you will be sent a paid invoice receipt with the details of the charges on the same day.

The credit card you supply will be kept on file in a secure server and used for subsequent pre-paid orders during your time with us.

Fees for pre-paid labs or supplements/products and corresponding shipping/handling fees must be processed in full before any lab kits and/or products are ordered and shipped.

Refunds for Pre-Paid Supplements, Labs, Shipping, and Handling Fees:

All fees for prepaid labs and/or supplement orders and corresponding shipping fees are non-refundable once the supplements have been shipped and/or the lab has been prepaid and sent out to the client. Refunds will only be extended if products were damaged in transit.

Ordering Functional Lab Workups:

As part of this coaching plan, I assist you in obtaining a set of self-order laboratory test results from various labs (Certified Laboratories in the United States and Europe) and assist you in understanding those test results about your specific health challenges that you can then use exclusively as an educational tool for personal health-building purposes.

Your doctor may use the same test results to diagnose and treat disease, but I do not do this.

The information I provide is not intended to, cannot, and should not be expected to substitute for a personal consultation with your own qualified doctor. I do not accept any liability for any failure to identify any medical condition or disease; this is not the purpose of the services offered.

I facilitate having the lab kits sent to your home, but you will then complete the kit and send payment directly to the lab unless Empowered To Heal, LLC must pay a lab fee in advance. As stated in the previous section, you will be billed for the lab's price and any corresponding shipping and/or handling fees before the lab kits are sent out.

Lab and Supplement Orders for International Clients Supplement Orders:

Additional fees for lab kits shipping to Canada and other international destinations will be incurred. The client is responsible for covering all additional shipping fees. Once the exact shipping fees are determined, depending on the client’s location in the world, the client’s credit card will be charged accordingly, as stated previously. Lab kits will not be sent out until all shipping fees and advanced fees (in the event lab fees must be paid upfront) have been paid in full.

Custom laws and what products are permitted to enter borders vary from country to country. Empowered To Heal LLC assumes no responsibility or liability for supplements/products that are detained and or confiscated by the country's customs department in which a client lives or has a product shipped to them. When placing supplement orders, clients assume full responsibility for all risks, custom fees, and shipping fees involved with supplement orders.

Scheduling Coaching Sessions:

STANDARD PLAN:

The frequency of coaching sessions will vary from client to client. When a coaching plan is booked, you'll be given a choice to have me schedule prearranged, recurring sessions on a regular day and time for you or handle all your bookings.

If you don't opt for me to schedule sessions on your behalf, you are responsible for and in control of scheduling coaching sessions via the online scheduling system that works best for you and per the availability.

Appointments must be scheduled at least one week in advance (to allow adequate time for preparation) and canceled at least 24 hours in advance, but I would appreciate at least 48 hours' notice if possible. Please keep this in mind when scheduling coaching sessions.

Note: It is advised to schedule several sessions in advance as time slots can fill up quickly, leaving little to no availability.

ULTRA PLAN:

Coaching sessions will be scheduled every two weeks on an agreed-upon regular day/time for a year with allowance for weekly sessions on occasion when strategic, up to a max of 30 sessions in 12 months from the date of purchase. Scheduling will NOT include weeks of major US holidays and/or scheduled vacation time.

There will be leeway to reschedule a day/time if a conflict arises, with the understanding that the schedule may not allow rescheduling in the same week.

Contact Outside of Scheduled Sessions and “Urgent” Situations:

Functional Health Consulting and Transformational Coaching are supportive and educational tools for improving nutrition, lifestyle, and overall well-being, so emergencies are unexpected.

Please call 911 and/or visit your local hospital for medical emergencies or other crises. For “urgent matters and questions,” you may reach out via messaging, but please understand that the scope of my services and work schedule does not allow me to be on-call or handle medical crises.

I will do my best to support you during challenging times within the scope of my services.

Cancellations and On-Time Appointments:

If you need to cancel or change an appointment time, you can do so through the online scheduling software. You do not need to call or email me because the software will alert me of the change.

You must cancel using the online scheduling system at least 24 hours in advance; otherwise, you will forfeit that appointment.

I understand that my clients have busy schedules, and I take pride in not keeping them waiting or keeping them longer than planned. I ask that you extend me the same courtesy by being on time for all scheduled consults.

90-Day Inactive Policy:

There is a 90-day Inactive Policy for all plans. After 90 days of no action taken on your part, which includes:

  • not scheduling sessions

  • not responding to emails

  • not submitting necessary intakes/documents

  • not implementing agreed-upon action steps

  • not sending an email letting us know you need more time or that you need to put your program on hold, your program becomes “inactive.”

  • Etc.

Once a program goes “inactive,” a re-enrollment fee of $500.00 will be needed to get started again. This is provided the coaching plan's expiration date is not past. If the expiration date has passed, then the coaching plan is expired and no longer eligible for the re-enrollment fee and/or scheduling sessions.

My time is limited, and I have specifically reserved a space for you in my busy schedule. If you are not ready to act, that is perfectly OK. I need to know so I can give the space to a ready client. When you are ready, just let me know, and I can make a new spot for you.

This policy does not apply to pay-as-you-go / stand-alone consults.

Ongoing Messaging Support:

If you have a question or concern between our scheduled coaching sessions that can’t be answered by doing your research, don't hesitate to get in touch with me via messaging. I am available to support you throughout the entire time you are an active, paying client.

I always intend to answer messages the same day they are sent, but please allow up to 72 hours. Messages are for brief support between consults; they are not intended to replace a consult. Additionally, please only expect messages to be answered M-F, not on the weekends.

I will respond to up to two short messages daily, provided they can be read and responded to in 5 minutes or less, and there are no more than two messages per day.

For example:

  • Can you clarify how I am supposed to take the zinc supplement you recommended? (with meals, 2x per day)

  • Can you give me a recommendation or two for panic attacks/acute anxiety? (Rescue Remedy from Bach Flower or Panic Button by Newton Homeopathics)

  • What is your top pick for a supplement for kidney stones? (Chanca Piedra Tea)

  • “I feel like I am backsliding big time. Should I schedule a session?” (Yes)

Messages requiring a detailed response, which will require a review of your client file and/or review of other information and can't be read and answered in 5 minutes or less, will require a scheduled consult.

For example:

  • Please review this video or link to a product and give me your feedback on my case.

  • I've attached my doctor's most recent lab results. Can you review them and give feedback on how I can improve the values?

  • I am feeling extremely discouraged right now. Do you have any suggestions for me to help me keep a healthy mindset?

Please do NOT send messages that you can easily do a Google search on your own and then wait to discuss your findings in a scheduled consult. Examples of such emails include:

  • Can you give me recommendations for exercises that I can do to stretch and strengthen my lower back?

  • I have a friend who just got diagnosed with cancer. What can she start doing diet and lifestyle-wise to better support her body?

  • I am struggling with increased anxiety. What self-care methods can I implement to help myself calm down?

Preparedness, Commitment, and Openness to Transformation:

Healing from complex, deeply rooted health conditions requires a great deal of commitment, openness to transformation, and preparedness on your part. You are in control of and responsible for your own outcomes related to the support and guidance I provide to you.

My role is to be a guide, mentor, and teacher who helps you create a road map for you to find your way back to health and wholeness. Nobody else can do the work for you. You must take full responsibility for your own healing journey and outcomes.

You are the one who must do the work and open your heart and mind to the transformation required to restore your health and reclaim your life. This is your journey, nobody else’s.

During the duration of your program, I will be sharing many important educational documents with you. It will be important to keep all files organized on your computer in a way that works for you. I also highly suggest that you print all documents off and organize them in a 3-ring binder so that you always have them accessible for reference during our consults and thereafter.

Guidance In Creating a Customized Healing Protocol:

During your program, I may make suggestions for targeted supplements. Supplements are not required and are not a prescription. As a functional health coach/practitioner, I do not diagnose, treat, cure, prevent, or manage disease.

Implementing supplements and any other suggestions is entirely your decision. Please consult with your doctor before implementing any suggestions, especially to ensure that they will not interact with any medications you may be taking. This is an important step in your program, as we want to be sure you have a safe experience.

When I discuss supplement options, I will go over sequencing and titration. This means introducing one supplement at a time, starting with the smallest dosage and building up over a few days before introducing the next one. If you were to take them all at once or start at that highest dosage and experience undesirable side effects, you would not know which one was triggering the problem. By careful sequencing and titration, you have the most control and understanding of what works for your unique body.

Session Prep Intakes:

To help me better prepare for our sessions, I require all clients to complete a brief online session prep questionnaire. The questionnaire will be sent to you at least two days in advance, and you are required to complete it no later than 24 hours before your scheduled consult. Failure to complete the session prep intake will result in the session being rescheduled to a later date.

Acknowledgment of Understanding:

I have read the Functional Health & Transformation Coaching Contract Agreement on the previous pages in full. I understood everything I have read and agree to all the terms described.
